Para Su Salud

A Good Idea

Description

Para Su Salud is a community benefit program of California Hospital Medical Center (CHMC). It was established in 2002 in order to increase access to health care for low-income, uninsured families living in CHMC’s service area. The program utilizes six bilingual (English and Spanish) Community Health Access Specialists (CHAS) who provide low-cost insurance information and counseling to low-income, uninsured families. CHAS conduct enrollment activities in community hospitals, clinics, health fairs, and other community organizations. CHAS also perform enrollment follow-up, troubleshooting, redetermination assistance, and retention contacts/verification.

The 2008 program goals were as follows: perform outreach to 7,500 of the target population; complete healthcare program applications for 3,500 clients; investigate enrollment status within three months of application completion date for 100% of clients; confirm enrollment for 75% of client applications; provide ongoing assistance to 2,500 clients; offer redetermination assistance to 75% of clients; and maintain a 75% retention rate for clients with confirmed enrollment.

This program is funded by the Los Angeles County Department of Health Services through its Children’s Health Outreach Initiative (CHOI).
