30-Day Hospital Readmission Rate: Medicare Population

This indicator shows the percentage of Medicare beneficiaries who were readmitted to a hospital within 30 days after being discharged. 
 
Medicare is the federal health insurance program for persons aged 65 years or older, persons under age 65 years with certain disabilities, and persons of any age with end-stage renal disease (ESRD).

Why is this important?

Hospital readmission occurs when a patient who has been discharged from a hospital is admitted again within a certain period of time. The standard benchmark used by the Centers for Medicare and Medicaid Services sets this period at 30 days after a previous discharge. Hospital readmission is used as a measure of a hospital’s quality of care and studies have shown that reducing hospital readmission rates can result in significant cost savings per patient.  Prioritizing this area encourages hospitals to expand their focus beyond the hospital setting and improve communication with patients after discharge.
