Preventing Progression to Type 2 Diabetes
Across the United States, chronic disease touches 1 in 2 adults with diabetes being one of the leading causes of death. According to the Centers for Disease Control and Prevention (CDC), more than 25 million people have diabetes, including both diagnosed and undiagnosed cases. Additionally, the direct medical expenditures attributable to diabetes are over $116 billion and equates for 86% of our nation’s health spending according to the CDC. Diabetes disproportionately affects minority populations and the elderly, and its incidence is likely to increase as minority populations grow and the U.S. population ages. Our mid-central Michigan region is witnessing an alarming rate of deteriorating health outcomes and preventable mortality, especially among the most vulnerable underserved communities.

About Good Measures: This innovative virtual program is designed to help people who have prediabetes or certain risk factors and want to try to prevent progressing to type 2 diabetes. Once enrolled, participants will have access to:
- Personalized Coaching - one-on-one coaching from their CDC-trained lifestyle coach—a clinician such as a registered dietitian or certified diabetes educator - that considers each participant’s age, gender, medical conditions, goals, barriers, food preferences, access to care, and socioeconomic status;
- Power Digital Technology - Good Measures’ technology enables participants to see how well they are meeting their goals and nutrient needs and to learn optimal portion sizes, access healthy recipes, and receive real-time meal and snack recommendations based on their current nutrient deficits and excesses; and
- The Good Measures Index (GMI): The GMI -- a number ranging from 0 to 100 -- gives participants a way to see their nutrition status, meal by meal, and learn to make better food choices to achieve their goals.
Participants also receive group member support from each other as they work on nutrition, physical activity, stress management, and weight loss. The sessions are offered as online group video at set times and or asynchronous online courses for participant convenience. Sessions are weekly for the first 16-weeks and gradually taper to monthly sessions in the last six months of the program year. The program helps individuals make positive changes in their eating and exercise behavior. It is particularly effective for people with prediabetes who also have one or more other complicating medical conditions.
The MiHIA Virtual DPP Good Measures program is available, free of charge, for patients aged 18 and older who live in the following counties: Arenac, Bay, Clare, Gladwin, Gratiot, Huron, Isabella, Iosco, Midland, Ogemaw, Roscommon, Saginaw, Sanilac, and Tuscola.
Other qualifications also include:

BMI greater or equal to 25 (23 if Asian)

No previous diagnosis of type 1 or type 2 diabetes

Blood test results in the prediabetes range within the past year:

Hemoglobin A1C: 5.7%-6.4% or
-
Fasting plasma glucose: 100-125mg/dL or
-
Two-hour plasma glucose (after a 75 gm glucose load): 140-199 mg/dL or

Previous diagnosis of gestational (pregnancy-related) diabetes
