Community Health Excellence Awards

Objective: Implementation of a widely-publicized and promoted health excellence award system to encourage healthy behaviors and to recognize the individuals and organizations that are serving as positive role models within the region on an annual basis.

The Michigan Health Improvement Alliance (MiHIA) Community Health Excellence Award recognizes and inspires outstanding contributions to improving the health status of the community in our 14-county region. Recognition is awarded to those groups or workplaces who have contributed to improving the culture of health in any capacity. Both evidenced-based and innovative actions will be considered for the award.This initiative was developed by MiHIA’s Population Health Strategy Team and aligned to the MiHIA Quadruple Aim Leadership Committee of the Board of Directors.

Significance

Organizations that choose to complete the MiHIA Health ScoreCard are recognized to be those that are, or inspire to, contribute to improving the health status of the community in our 14-county region. Completing the Health ScoreCard will enable you and your organization to:

  • Build an understanding of your organization and how you are impacting health in the region.
  • Become part of a network of organizations committed to improving health for the region. This network can provide your organization with access to the latest in best practices for employee wellness and opportunities to collaborate and innovate with regional partners.
  • Gain access to the network of collaborative technical assistance, data, and information on health improvement.
  • Identify opportunities to connect you to the network of available resources and identify opportunities for your organization to contribute to the body of knowledge we have in the region.
  • Enable your organization identify evidence-based health improvement strategies that could be implemented within your organization

Goals and Key Strategies

GOAL 1: Receive at least 20 applications for the CHEA

  • Key Strategy 1: Encourage and promote application completion to all organizations within the MiHIA network 
  • Key Strategy 2: Notify previous CHEA applicants of the 2018 CHEA; promote application completion
STATUS: In Progress - 7 submissions have been received as of December 10, 2018
 

GOAL 2: Receive at least one application per county in the MiHIA region

Note: Application from organizations that serve multiple counties will be counted for each county they represent, as long as those policies/services apply to all sites)
  • Key Strategy: Encourage and promote application completion to all organizations within the 14-county MiHIA region
STATUS: In Progress - 10 counties (Clare, Gladwin, Gratiot, Huron, Isabella, Midland, Ogemaw, Saginaw, Sanilac, and Tuscola) have been represented per submissions received as of December 10, 2018
 

GOAL 3: The organizations that apply will represent at least four different business sectors (Ex: education, healthcare, hospitality, etc.)

  • Key Strategy: Encourage and promote application completion to all organizations within the MiHIA network
STATUS: Goal Met - 4 sectors (healthcare system, large employer, non-profit, and public health) have been represented per submissions received as of October 1, 2018
 

GOAL 4: Of the applications received, 30% will achieve Bronze Status

  • Key Strategy: Upon receiving initial applications, community champions will and provide resources for easy and budget-friendly actionable interventions which can be implemented to improve final scores
STATUS: Not Started - Achieved status' will be determined following the closing of the 2018 submission cycle
