Goal: The goal of this program is to educate during every visit, to assist patients in developing a longitudinal personal record of medical history and care plans, and to provide a tangible way to engage patients in their own care. The overarching goal was better compliance, recognition of medication side effects, and improved adherence to specific and agreed upon lifestyle changes.

Impact: Sixty percent of patients participate in care plan tracking with a health notebook, and 80% percent of patients complete a prep form to help organize visits.

Goal: To provide transportation to patients in South Central Missouri who otherwise have non-existent, limited, or expensive transportation options to and from healthcare appointments.

Impact: HealthTran has helped to improve healthcare access and long-term health outcomes, as well as reduced preventable hospitalizations and unnecessary emergency department visits.

Goal: The goal of the Healthy Hometown Restaurant Initiative is to encourage Louisville restaurants to provide and promote healthier meal options.

Impact: Participants that have volunteered for the Healthy Hometown Restaurant Initiative include 16 restaurants with 33 locations and one caterer.

Goal: Healthy Howard opens doors for individuals, groups and communities to improve their health and wellness.

Impact: Healthy Howard provides resources to schools, restaurants, workplaces, and childcare centers to help them improve the health and choices of their students, employees, or clients.

Goal: The Jamaica Service Program for Older Adults mission is "a total community working together to provide services to, and develop skills in, older adults in order to promote independence and enhance their quality of life."

Goal: MHHM was rolled out with the following overall goal: to create a community wide culture that encourages and supports healthy lifestyles by promoting increased physical activity (10,000 steps/day goal), optimal nutrition, healthy public policy and access to resources and facilities that bolster the stated goal. Specific goals include:

- Increasing the number of people in Louisville Metro who engage in 30 minutes of moderate physical activity at least 5 days a week by 15%.
- Decreasing the percentage of overweight or obese people in Louisville Metro by 10%.
- Increasing from 22% to 38% the number of people in Louisville Metro who eat five or more servings of fruits and vegetables per day.

Past and future Behavior Risk Factor Surveillance System (BRFSS) surveys will be used to measure baseline and follow-up indicators.

Michigan Health Improvement Alliance
