The Mobile C.A.R.E. Foundation’s mission is to provide complimentary and comprehensive asthma care and education to children and families in Chicago’s underserved communities via mobile medical units called "Asthma Vans."
The Mobile C.A.R.E. Foundation’s Asthma Vans provide children and families in Chicago’s underserved communities with complimentary and comprehensive asthma care and education resulting in reduced school absenteeism, decreased ER visits and lower hospitalization rates.

The Northern Michigan Diabetes Initiative is a regional collaboration dedicated to prevention, early detection, and management of diabetes. The Healthy Family Backpack Program connects with youth and their parents to educate participants on proper nutrition and promote healthy lifestyles to reduce childhood obesity.
The Northern Michigan Diabetes Initiative has distributed nutritional education materials to over 300 families. Of ninety-two families that set a healthy goal at the start of the program, forty-five continued to maintain that goal at the two-month mark.

Clothing & Connections' mission is to provide interview and career clothing, information, and referrals to clients in need as they pursue new opportunities for advancement in self-sufficiency. Clients are referred to Clothing & Connections by a group of non-profit member organizations, which include homeless shelters, domestic violence centers, job training programs, and self-sufficiency programs. Clients are fitted with clothing specific to the job they are applying for. In addition, Clothing & Connections offers mini-seminars on topics such as preparing for the job interview, making a positive first impression, and dressing for success on a limited budget.

The mission of Reconciliation Services is to cultivate a community seeking reconciliation to transform Troost from a dividing line into a gathering place, revealing the strength of all. Our mental health services are at the heart of how we strive to help clients see their own strength and work towards being reconciled to their own health and well-being.
Over the past three years, our therapy program participants reported a 71 percent decrease for depression and 79 percent decrease for PTSD.
