Pathways to Population Health
Objective(s): To make practical, meaningful, and sustainable advances in population health
Pathways to Population Health (P2PH) is the result of collaboration amongst hundreds of organizations and change agents united by a common purpose: to leverage shared assets and unique strengths to help health care organizations accelerate their individual and collective population health improvement efforts.
P2PH is the product of collaboration among five organizations: American Hospital Association/Health Research and Educational Trust, Institute for Healthcare Improvement, Network for Regional Healthcare Improvement, Public Health Institute, and Stakeholder Health. They are honored to be joined by a number of organizations serving as Pioneer Sponsors of the P2PH movement, leading the way to spread the word and engage others in this movement.

The Compass is ideally suited for individuals or teams with agency to advance population health improvement efforts within their organization. It takes approximately 20 minutes to complete. It helps you see the balance of your current activities within and across portfolios, and identify areas you may wish to focus on to accelerate your population health improvement efforts. We recommend using the Compass quarterly to assess your progress over time.
