Medicare Hospital Readmissions: Heart Failure

This indicator shows readmissions within 30 days for Medicare fee-for-service Part A beneficiaries, ages 18 and older, with a principal diagnosis of heart failure.

Why is this important?

Heart failure (HF) is a growing healthcare burden and one of the leading causes of hospitalizations and readmissions. Approximately 5.7 million American adults are living with heart failure and the projections are that the prevalence of HF will increase 46% from 2012 to 2030, with greater than 8 million adults living with the chronic condition. Readmissions due to heart failure receive attention from policy makers as they are perceived as a correctable source of poor quality of care and excessive medical spending.
